All students entering 6th or from out of state are required to have a current physical examination completed on the approved Illinois Department of Public Health Form and up to date Immunizations.
A sports physical does NOT meet the requirement
A current physical is one that was completed within one year of the first day the school year
If a current physical and upto date immunizations are not turned in by the 3rd Monday of September or within 30 days of entering school, the student will be excluded from school until the required paperwork is turned in
A student should not have medication on them. In the event that a student may carry and self administer medication (inhaler or Epipen) the proper form must be filled out by the parent/guardian and student
A parent may come during the school day to administer medication to student
For the school nurse to administer medication, the parent/guardian of the student will be responsible for having the student's doctor fill out the Medicine Release Form and turning it into the school nurse. The Medication Authorization Release Form must be completed for each school year
The parent/guardian of the student is responsible for supplying the medication; The medication must be brought in its original container and properly labeled
